About Supportability

Our Story

Supportability is a specialist Support Coordination service with deep foundations in acquired disability, rehabilitation and complex community practice.

Our story began with Rehability Australia, which has been serving the disability and rehabilitation sector since 2004.

Over more than two decades, Rehability Australia developed extensive experience working alongside people living with acquired brain injury, spinal cord injury, neurological conditions, catastrophic injury and other complex acquired disabilities.

As the NDIS developed, Support Coordination became an increasingly important part of this work.

In 2025, Supportability was launched as an independent arm of the business, creating a dedicated service focused on Support Coordination and Specialist Support Coordination.

While Supportability has its own identity and focus, the experience behind it has been developing since 2004.

Founded on Social Work Principles

Our approach is grounded in the social work principles and values that have underpinned our work for more than two decades.

We understand people within the context of their whole lives — their strengths, relationships, environment, culture, community, rights, goals and the systems surrounding them.

Our practice is guided by:

  • person-centred and strengths-based practice
  • human rights, dignity and respect
  • self-determination, choice and control
  • supported decision-making
  • safeguarding
  • social inclusion and participation
  • professional integrity and accountability
  • collaboration and partnership
  • evidence-informed practice.

Not every member of the Supportability team is a social worker. However, these principles provide the foundation for our organisational practice framework and influence how our entire team approaches Support Coordination.

Our Approach

We believe good Support Coordination should do more than connect people with services.

It should help people understand their options, make informed decisions, strengthen their support networks and develop arrangements that can work sustainably over time.

Our approach therefore focuses on:

Understanding the whole person
We consider disability alongside the person’s environment, relationships, health, strengths, preferences and goals.

Building capacity
We work to strengthen the participant’s understanding, confidence and ability to make decisions and navigate their supports.

Connecting systems
Where multiple services are involved, we help establish communication, clarify responsibilities and reduce fragmentation.

Safeguarding
We remain alert to vulnerability, risk and emerging instability while respecting the participant’s rights, dignity and autonomy.

Working collaboratively
We work alongside participants, families, providers, clinicians and other professionals rather than attempting to replace their roles.

Independent by Design

Supportability is independent of direct personal care, SIL and SDA service delivery.

This independence allows us to explore options based on the participant’s circumstances, preferences and goals.

We work collaboratively with providers while remaining focused on the participant and the sustainability of their overall support arrangements.

Looking Forward

The disability sector continues to evolve, and Supportability will evolve with it.

Our commitment is to maintain the specialist expertise, professional foundations, quality and independence that have shaped our work from the beginning.

Whatever the future of disability support looks like, our purpose remains clear:

Understand the whole person. Connect the right supports. Navigate complexity. Build sustainable outcomes.
