Our Services
Supportability provides Support Coordination and Specialist Support Coordination for NDIS participants.
Our services are tailored to the person’s circumstances, goals and level of complexity.
Support Coordination
Our Support Coordinators help participants understand and implement their NDIS plans, connect with appropriate services and strengthen their ability to manage their supports.
This may include:
- understanding the NDIS plan and available supports
- identifying and connecting with providers
- establishing services and service agreements
- coordinating multidisciplinary supports
- connecting with mainstream and community services
- addressing barriers affecting plan implementation
- preparing for plan reassessment
- strengthening informal and community supports
- responding when existing arrangements are not working.
Our aim is not to create unnecessary dependence on Support Coordination.
We work to strengthen the participant’s confidence, knowledge and support network so that their arrangements become increasingly sustainable.
Specialist Support Coordination
Specialist Support Coordination may be appropriate where significant complexity or barriers are affecting a participant’s ability to establish or maintain their supports.
This can include situations involving:
- complex disability
- multiple service systems
- significant safeguarding concerns
- repeated provider breakdown
- behavioural complexity
- hospital discharge
- accommodation instability
- complex health needs
- impaired decision-making
- crisis or significant life transition.
We work to understand what is contributing to the complexity and help establish a more coordinated and sustainable support environment.
